Associations between technology use, perceived burdensomeness, and independence among rural older adults

    Bal, Betül; Apaydin, Emine

Objective: This study aims to examine the relationships among perceived burdensomeness, technology use habits, and activities of daily living (ADL) in older adults residing in rural areas. Methods: This descriptive correlational study was conducted with 384 older adults living in rural areas in two different regions of Turkey. Data were collected using an Information Form, the Geriatric Perceived Burdensomeness Scale, the Barthel Index for Activities of Daily Living, and the Technology Use Habits Scale. Results: Perceived burdensomeness was moderately and negatively correlated with ADL (r = − 0.484, p < 0.001), whereas technology use habits were weakly but positively correlated with BADL (r = 0.192, p < 0.001). In the hierarchical regression analysis, technology use habits showed a positive and statistically significant association with ADL in the initial model (β = 0.189, p < 0.001). When perceived burdensomeness was added to the model, it emerged as a strong negative predictor of ADL (β = −0.497, p < 0.001), substantially increasing the explained variance. In the fully adjusted model, perceived burdensomeness, age, digital device use, and duration of digital device use were significantly associated with ADL, whereas age did not significantly affect the relationship between perceived burdensomeness and ADL. Overall, perceived burdensomeness and technology use habits were identified as key factors related to functional independence among rural older adults. Conclusion: The findings of this study indicate that perceived burdensomeness and technology use habits are closely associated with independence in activities of daily living among older adults living in rural areas. These findings indicate an association between technology use and perceived burdensomeness and provide a foundation for future research in nursing practice and rural health services. Future research is recommended to focus on evaluating technology-based and psychosocial interventions that support independence and psychosocial well-being in rural older populations. Clinical trial number: Not applicable.

Perceived burdensomeness and happiness in cohabiting older adults: The serial mediating roles of mental well-being and loneliness

Akeren, Zahide; Akeren, Ihsan; Kalyoncuo, Servet

Background Aging-related changes and increased dependence on family members may negatively affect older adults’ psychological well-being. Perceived burdensomeness refers to the belief that one is a burden on others and has been associated with lower happiness, reduced mental well-being, and greater loneliness. Among adults living with their families, mental well-being and loneliness may explain how perceived burdensomeness relates to happiness in later life. Purpose This study examined the associations between perceived burdensomeness and happiness among older adults living with their families and evaluated the serial mediating roles of mental well-being and loneliness. Methods This cross-sectional study included individuals aged ≥65 years residing with family members in a city in Turkey’s Eastern Black Sea region. Data were collected from 273 older adults; after preliminary screening, 7 cases were excluded, resulting in a final analytic sample of N = 266. Measures included the Geriatric Feelings of Burdensomeness Scale, WHO-5, the Happiness Scale, and the Loneliness Scale for the Elderly. Serial mediation was tested using a regression-based approach with bootstrap confidence intervals. Results Perceived burdensomeness was associated with lower mental well-being and higher loneliness. The total association between burdensomeness and happiness was negative (β=-0.467), and more than two-thirds of this association operated indirectly through mental well-being and loneliness. Conclusion and Implications Perceived burdensomeness was associated with lower happiness among older adults living with their families, primarily through reduced mental well-being and increased loneliness. In geriatric nursing and primary care, routinely assessing these psychosocial factors and implementing family-centered and social support-oriented interventions may help improve happiness.

Fear of Falling Avoidance Behavior on Daily Living Activities and Physical Activity in Older Adults: A Cross-Sectional Study

Türkmen Keskin, Sultan; Sönmez Sari, Ebru; Canbulat, Şahinde; Öztürk, Fatma Özlem

The aim of this study was to investigate the effect of avoidance behavior due to fear of falling on dependency levels in activities of daily living and physical activity levels in older adults. This study is a cross-sectional research design. Data were collected between November 2023 and March 2024. A total of 506 individuals aged 65 years and over participated in the study (mean age 71.26). It was determined that the older adults had low level of fear of falling avoidance behaviors were fully independent in activities of daily living and had low level of physical activity. The multiple linear regression analysis showed that age, marital status, education, living with and fear of falling avoidance behavior affected daily living activities; age, place of residence, gender, income status, chronic disease status, and fear of falling avoidance behavior affected physical activity level (R2 = 0.183; R2 = 0.197, respectively). This study revealed the necessity of interventions to prevent fear of falling avoidance behaviors in order to increase the activities of daily living and physical activity levels among older adults.

Effect of geriatric massage in elderly patients with type 2 diabetes: A randomized controlled trial

    Apaydın, Emine; Hintistan, Sevilay

Objective: This randomized controlled trial examined the effects of geriatric massage on diabetes-related symptoms and blood parameters in older adults with type 2 diabetes. The study included 59 participants (29 in the geriatric massage group and 30 in the control group). Methods: Data were collected using the Older Adults Information Form, the Diabetes Symptoms Checklist, and the Blood Parameters Monitoring Form. Participants in the intervention group received 30-minute geriatric massage sessions twice weekly for 12 weeks. Statistical analyses included the Shapiro–Wilk test, Mann–Whitney U test, ANOVA, Friedman test, Wilcoxon test, Chi-square test, Fisher–Freeman–Halton test, Greenhouse–Geisser correction, Bonferroni adjustment, and paired and independent t-tests. Results: Compared with the control group, participants receiving geriatric massage showed a significant reduction in diabetes symptoms (p < 0.001), an increase in HDL cholesterol levels, and decreases in other blood parameters (p < 0.05). Discussion: Geriatric massage may help reduce diabetes symptoms and improve selected blood parameters in older adults with type 2 diabetes.

Breaking the silence on late-life depression: uncovering the drivers of help-seeking in Türkiye’s ageing population

Ercan, Uğur; Apaydın, Emine; Alkan, Ömer

Background: Due to the prevalence of physical health issues among older adults, depression remains a significant health concern, which is often overlooked. Understanding the factors influencing the likelihood of seeking psychological support plays an important role in effectively managing depression in later life, where psychological support has a crucial role. This study aims to examine the factors associated with the likelihood of receiving psychological support among individuals aged 60 years and older experiencing depression in Türkiye. Methods: This study used microdata from the Türkiye Health Surveys conducted by Turkish Statistical Institute in 2016, 2019, and 2022. Gompit, binary logistic regression, and CHAID analyses were used to identify variables associated with psychological support-seeking behavior among older adults with depression. Results: The analysis revealed that general health status, illness status, gender, education level, and difficulty in healthcare payment significantly influenced the likelihood of receiving psychological support among older adults with depression. Conclusion: The results achieved in this study indicate that among older adults experiencing depression, women, those with higher educational levels, and those with existing illnesses are more likely to seek psychological support. Based on these results, it is recommended to develop educational programs to raise societal awareness about the importance of psychological support and to inform health policies accordingly.

The effect of nurse-led motivational interviewing based on the trans-theoretical model on promoting physical activity in healthy older adults: A randomized controlled trial

    Sönmez Sari, Ebru; Kitiş, Yeter

Aim: The aim of this study was to evaluate the effect of motivational interviewing based on the trans-theoretical model promoting physical activity in older adults. Methods: A randomized controlled trial study was conducted with 117 older adults (intervention group: 58, and control group: 59) between April and November 2019. The data were collected via a questionnaire, trans-theoretical model scales, the Physical Activity Scale for the Elderly, the Katz Activities of Daily Living scale and a pedometer. The 24-week intervention consisted of regular motivational interviewing. The control group received usual care at the family health centre. Results: Following the intervention, the Intervention Group showed significant improvements Exercise Processes of Change Scale, Exercise Self-Efficacy Scale and Perceived Benefits of the Decisional Balance Scale for Exercise. There was a significant difference between the groups in terms of stages of change. In the Intervention Group, 81.5% were in the contemplation stage in the pre-test, while in the post-test, 70.4% had transitioned to the action stage. In contrast, in the control group, 62.5% were in the contemplation stage initially, but in the post-test, only 9.3% had reached the action stage. The mean number of step counts increased significantly in favour of the Intervention Group, as did the Physical Activity Scale for the Elderly total score. Conclusion: The trial indicated that after the trans-theoretical model-based motivational interviewing, the stages of change among the older adults improved, as did their exercise behaviours. It is recommended that the trans-theoretical model and motivational interviewing be used by nurses to improve healthy lifestyle stage behaviours in older adults.
