Development and psychometric evaluation of the Self-Regulated Nursing Care Scale (SRNCS) among Turkish nurses

Akeren, Zahide; Akeren, İhsan; Isik, Hafsa Kubra

Background: Self-regulation refers to individuals’ ability to control themselves, manage their behaviours and learn from their environment. Self-regulated learning skills may support nurses in providing effective care. This study aimed to develop and psychometrically evaluate the Self-Regulated Nursing Care Scale (SRNCS) for assessing nurses’ self-regulated nursing care skills based on Zimmerman’s Self-Regulated Learning Model (Z’SLM) and the relevant literature. Methods: The planning, implementation and evaluation components of nursing care were considered in relation to the forethought, performance and self-reflection phases of Z’SLM. Data were collected online between November 2022 and January 2023 from nurses working in state hospitals. Content validity was assessed through expert review. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were performed using data from 201 and 239 nurses, respectively. Criterion validity and reliability were also examined. Results: EFA yielded a 16-item, three-dimensional structure comprising planning, implementation, and evaluation and explaining 63.9% of the total variance. CFA supported this three-factor structure in an independent sample. Criterion validity was supported by correlations with the Counselling Skills in Nurses Scale. Cronbach’s alpha was 0.95 for the overall scale and 0.81, 0.87, and 0.93 for the planning, implementation, and evaluation dimensions, respectively. The test-retest reliability coefficient was 0.77. Conclusion: The findings support the validity and reliability of the Turkish form of the SRNCS for assessing self-regulated care skills among nurses providing inpatient care. The scale may be used in research and clinical settings to assess planning, implementation and evaluation dimensions of self-regulated nursing care. Future studies may examine relationships between SRNCS scores and care-related outcomes.

Perceived burdensomeness and happiness in cohabiting older adults: The serial mediating roles of mental well-being and loneliness

Akeren, Zahide; Akeren, Ihsan; Kalyoncuo, Servet

Background Aging-related changes and increased dependence on family members may negatively affect older adults’ psychological well-being. Perceived burdensomeness refers to the belief that one is a burden on others and has been associated with lower happiness, reduced mental well-being, and greater loneliness. Among adults living with their families, mental well-being and loneliness may explain how perceived burdensomeness relates to happiness in later life. Purpose This study examined the associations between perceived burdensomeness and happiness among older adults living with their families and evaluated the serial mediating roles of mental well-being and loneliness. Methods This cross-sectional study included individuals aged ≥65 years residing with family members in a city in Turkey’s Eastern Black Sea region. Data were collected from 273 older adults; after preliminary screening, 7 cases were excluded, resulting in a final analytic sample of N = 266. Measures included the Geriatric Feelings of Burdensomeness Scale, WHO-5, the Happiness Scale, and the Loneliness Scale for the Elderly. Serial mediation was tested using a regression-based approach with bootstrap confidence intervals. Results Perceived burdensomeness was associated with lower mental well-being and higher loneliness. The total association between burdensomeness and happiness was negative (β=-0.467), and more than two-thirds of this association operated indirectly through mental well-being and loneliness. Conclusion and Implications Perceived burdensomeness was associated with lower happiness among older adults living with their families, primarily through reduced mental well-being and increased loneliness. In geriatric nursing and primary care, routinely assessing these psychosocial factors and implementing family-centered and social support-oriented interventions may help improve happiness.

Self-regulation in blood glucose management among middle-aged and older adults with type 2 diabetes in a rural and semi-rural setting: A qualitative study

    Akeren, İhsan; Akeren, Zahide

Type 2 diabetes mellitus (T2DM) is a chronic metabolic disease with increasing prevalence, particularly among middle-aged and older adults. Effective blood glucose management is essential for preventing complications and improving quality of life. Although self-regulation has been widely examined in diabetes management, little is known about how middle-aged and older adults living in rural and semi-rural settings apply self-regulation strategies in daily life. This qualitative study explored the blood glucose management experiences of individuals aged 50 years and older with T2DM using Zimmerman’s cyclical self-regulated learning model. Face-to-face interviews were conducted with 16 hospitalized participants meeting the rural or semi-rural residence criterion. Data were analyzed using an inductive-deductive hybrid thematic approach and organized with MAXQDA 20. Three main themes and 13 subthemes were identified: preparation and awareness, barriers and management behaviors, and self-evaluation and internal awareness. During the forethought phase, participants described knowledge, motivation, awareness, and planning but also reported misconceptions about diet and blood glucose management. In the performance phase, behavioral, emotional, cognitive, and social barriers, comorbidities, and poor adherence to dietary recommendations were common. During self-reflection, participants recognized physical symptoms and emotional influences but rarely reported systematically evaluating outcomes or modifying subsequent management behaviors. The findings indicate that inadequate knowledge, emotional challenges, comorbidities, limited social support, and ineffective coping strategies may disrupt the cyclical self-regulation process. Educational interventions for middle-aged and older adults in rural and semi-rural settings should strengthen knowledge, support self-management behaviors, and promote evaluation of outcomes and adaptation of future actions.

Metabolic syndrome index measurement tool (MSI): scale development, reliability and validity study

    Akeren, Zahide; Apaydın, Emine

Aim: Identifying the risks of metabolic syndrome (MetS) can lead to early targeted interventions and thus contribute to improved quality of life by reducing the risk of developing MetS, diabetes or heart disease in the future. We aimed to develop a valid and reliable measurement tool to measure the MetS risk of the population. Materials and methods: In the methodological study, an item pool was created by reviewing the literature. Pre-application was performed after the weighting of the items whose content validity was ensured by taking expert opinions. Data were collected from 43 patients with MetS from a state hospital affiliated to the Ministry of Health and 405 individuals without MetS from the community, from a total of 448 individuals using the Individual Information Form, Finnish Diabetes Risk Scale (FINDRISC) and Metabolic Syndrome Index (MSI). The data obtained were evaluated using SPSS 22.0 and MedCalc 19.1 statistical programmes. Scale discrimination was analyzed by independent samples t-test between the upper and lower 27% groups. The cut-off point of the scale score in predicting the diagnosis of MetS was tested by ROC analysis. Correlation analysis was performed with the parallel form for criterion validity. Results: As a result of the ROC analysis, a perfectly compatible scale with a sensitivity of 100%, a specificity of 85.43% and a cut-off score of 48 was obtained. When the correlation analyses between MSI and FINDRISC scores were examined for criterion validity, a positive moderate (r = 0.632, p < 0.001) correlation was found between FINDRISC and MSI. When the discrimination of the scale was analysed, it was found that there was a significant difference between the lower 27% and upper 27% groups (p < 0.05) and it was revealed that the MSI made sensitive measurements to discriminate. Conclusions: The MSI scale is a valid and reliable tool for early detection of MetS risk.

    Akeren, Zahide; Akeren, Ihsan

The study aimed to examine the relationships between loneliness, perceived support and fear of old age, to determine how they differ according to some socio-demographic variables and of loneliness to reveal the mediating role of perceived social support in the effect on fear of old age. In the descriptive cross-sectional study conducted between November 2023 and January 2024, the Loneliness Orientation Scale (LOS), Multidimensional Perceived Social Support Scale (MSPSS) and Fear of Old Age Scale (FOS) were used. Correlational analysis was used in the analysis of data collected from 611 participants aged 18–65 with two-stage sampling, T-test and ANOVA were used in the comparison of means, and the Hayes’ PROCESS macro was used to determine the mediating role of perceived social support. Women, singles, young people, those with low education and income fear old age more. There is a positive correlation between loneliness and fear of aging, and a negative correlation between these two and perceived social support. As a result, perceived social support is a partial mediator in the effect of loneliness on fear of old age and it should be considered as an effective source in reducing fear of old age.

    Akeren, Zahide; Hintistan, Sevilay

Background: Mild cognitive impairment can be considered as the preclinical stage of Alzheimer's disease. With the addition of COVID-19 disease to the cognitive decline that occurs with aging, it may cause dementia with severe cognitive losses such as Alzheimer's disease in some people. Physical activity and brain-occupying practices can significantly improve the risk factors for cognitive disorders and Alzheimer's disease. Objective: The aim of study was to examine the effect of green walking and intelligence game on the cognitive skills of individuals aged 50–70 years with COVID-19. Methods: The randomized controlled study was completed with 40 individuals, 20 in the experimental and 20 in the control group. Individuals in the experimental group green walking for 30 min three days a week for one month in two groups of 10 people, accompanied by the researcher. In addition, these individuals played intelligence games (brain games-memory thinking and brain exercise) via their smartphones for 15 min every day for a month. Results: The cognitive skills and cognitive dimensions of “attention, concentration, memory, language and orientation” of the individuals in the experimental group were significantly increased compared to the control group individuals (p < 0.05). Conclusions: It was found that green walk and intelligence game practice increased the cognitive skills of individuals aged 50–70 years who had outpatient COVID-19. © The Author(s) 2024. Investigation of university students' exposure to phubbing and being phubbed

Kırbaş, Zila Özlem; Semerci, Vahide; Güneş, Hüseyin; Sönmez Sarı, Ebru; Akeren, Zahide

Aim: This study was carried out as an investigation of university students' exposure to phubbing and being phubbed. Method: The sample of the descriptive and relationship-seeking study consisted of students studying at a university located in the Eastern Black Sea region of Turkey (n=1005). The data were collected online by the researchers between September and October 2022 using the Personal Information Form, the General Exposure to Phubbing (GEP) Scale, and the General Being Phubbed (GBP) Scale. Results: The average age of the students was 20.41±2.02 (min=18, max=36). 71.4% of them were female students, 67.5% of them were studying at the faculty and 56.6% were in the first year. The mean GEP score of the students was 77.86±26.28 and the mean GBP score was 48.48±18.52. According to the results of the Pearson correlation test, which was conducted to determine the relationship between students' exposure to phubbing and being phubbed, a moderate, positive linear relationship was found between GEP and GBP (r=0.418, p=0.000). Conclusion: As a result, it can be said that the GEP and GBP status of the students are moderate. In addition, students who are exposed to phubbing may exhibit more phubbed behaviors. It can be suggested that healthcare professionals, who have an important role in improving public health, should conduct effective identification and awareness studies for university students, who constitute an important risk group in terms of phubbing.
