- MakaleAçık Erişim2025Scopus SJRQ1Experiences of adults with type 2 diabetes mellitus with low socioeconomic status: a qualitative study
Semerci Çakmak, Vahide; Sönmez Sari, Ebru; Çetinkaya Özdemir, Serap
Background: Adults with diabetes encounter various challenges related to managing their condition. In this study, we explored the experiences of adults with type 2 diabetes mellitus with low socioeconomic status in Türkiye. Methods: This study was conducted as a phenomenological qualitative research. The purposive sampling method was used to select adults with type 2 diabetes. Data saturation was achieved with 20 patients. The data were collected through a demographic information form and a semi-structured questionnaire. The participants were interviewed by phone. The interviews were conducted in a quiet environment where the participants could respond comfortably. During the data analysis process, the content analysis approach suggested by Graneheim and Lundman (2004) was used. This method involves identifying meaningful units within the text, coding these units, and developing sub-themes and main themes. The MAXQDA 20 software was used for data management. Results: The participants were between 41 and 71 years old. Four main themes and ten subthemes were identified in the analysis. Themes: The identified themes included difficulties with dietary management of diabetes, individual difficulties, difficulties with self-management in diabetes, and coping strategies. According to Theme 1, it was observed that the participants experienced difficulties in managing nutrition in diabetes due to the economic aspect of diet and the inability to adapt to the diet. According to Theme 2, it was observed that the participants faced physical, psychological, and social difficulties. According to Theme 3, it was observed that the participants faced difficulties related to diabetes self-management, including blood glucose monitoring, insulin injection administration, and maintaining glycemic control.” According to Theme 4, it was observed that the participants attempted to cope with diabetes-related difficulties through healthy lifestyle habits and family support. Conclusion: Our findings indicate that adults with type 2 diabetes in low socioeconomic status face challenges related to biopsychosocial factors, nutrition, and diabetes self-management. However, these individuals managed their difficulties by adopting healthy lifestyle behaviors and relying on family support. The challenges faced by adults with diabetes may be alleviated through awareness training on dietary management and glycemic control.

Sönmez Sari, Ebru; Semerci Çakmak, Vahide; Çetinkaya Özdemir, Serap; Eşsiz Sefer, Betül
Aims: This study had three aims: to determine the hypoglycaemic confidence levels of participants, to identify factors affecting hypoglycaemic confidence levels of participants and to assess experiences related to hypoglycaemia of participants. Design: The explanatory–sequential–mixed method was used in this study. Methods: The quantitative stage included a sample of 177 people, and the qualitative stage included a sample of 18 people. Data of the study were collected between April and June 2023 with the Hypoglycemic Confidence Level Scale, Personal Information Form and Semi-Structured Interview Form. Descriptive statistics, independent samples t-test, one-way ANOVA and multiple linear regression analysis were used for quantitative data analysis. For the qualitative data analysis, content analysis was performed in the MaxQda program. Results: The scale items are scored between 1 and 4, and an increase in the score obtained from the scale indicates an increase in confidence levels. The mean hypoglycaemic confidence level score of the participants was 3. The data obtained in the qualitative stage were grouped under three main themes: Experiences Related to Hypoglycaemia, Reasons for Experiencing Hypoglycaemia and Managing Hypoglycaemia. Conclusion: Identifying hypoglycaemic confidence levels and hypoglycaemia experiences of patients with diabetes can guide health professionals, especially nurses, in promoting person-centred care interventions. Impact: The study discussed the hypoglycaemic confidence levels of diabetic patients and their experiences related to hypoglycaemia. The factors affecting the hypoglycaemic confidence level of the participants were educational status, income status, fear of experiencing hypoglycaemia, ability to self-administer insulin, receiving hypoglycaemia training and frequency of experiencing hypoglycaemia. The results of this study may provide guidance for the development of appropriate prevention and coping strategies for hypoglycaemia. Identifying the experiences of patients with diabetes with hypoglycaemia can guide health professionals, especially nurses, in promoting person-centred care interventions. Reporting Method: The Good Reporting of a Mixed Methods Study (GRAMMS) checklist was used for reporting. Patient or Public Contribution: No patient or public contribution.

- MakaleYalnız künye2024Scopus SJRQ2A dual-acting aldose reductase inhibitor impedes oxidative and carbonyl stress in tissues of fructose- and streptozotocin-induced rats: comparison with antioxidant stobadine

Inhibiting aldose reductase (ALR2, AR) as well as maintaining a concomitant antioxidant (AO) activity via dual-acting agents may be a rational approach to prevent cellular glucotoxicity and at least delay the progression of diabetes mellitus (DM). This study was aimed at evaluating the dual-acting AR inhibitor (ARI) cemtirestat (CMTI) on tissue oxidative stress (OS) and carbonyl stress (CS) biomarkers in rats exposed to fructose alone (F) or fructose plus streptozotocin (D; type-2 diabetic). D and F rats were either untreated or treated daily with low- or high-dose CMTI, ARI drug epalrestat (EPA) or antioxidant stobadine (STB) for 14 weeks. Malondialdehyde (MDA), glutathione S-transferase (GST), nitric oxide synthase (NOS), and catalase (CAT) were increased in the sciatic nerve of F and D. These increases were attenuated by low doses of CMTI and STB in D, but exacerbated by low-dose EPA and high-dose CMTI in F. STB and CMTI and to a lesser extent EPA improved MDA, protein-carbonyl, GST and CAT in the hearts and lungs of F and D. CMTI and STB were more effective than EPA in improving the increased MDA and protein-carbonyl levels in the kidneys of F and especially D. CMTI ameliorated renal GST inhibition in D. In the lungs, hearts, and kidneys of F and D, the GSH to GSSG ratio decreased and caspase-3 activity increased, but partially resolved with treatments. In conclusion, CMTI with ARI/AO activity may be advantageous in overcoming OS, CS, and their undesirable consequences, with low dose efficacy and limited toxicity, compared to ARI or antioxidant alone.

The therapeutic potential of vortioxetine on mechanical hyperalgesia/allodynia was investigated in rats with streptozotocin-induced diabetes, and its possible mechanism of action was elucidated in this study. The obtained findings demonstrated that subacute vortioxetine treatment (5 and 10 mg/kg for 2 weeks) increased the reduced paw-withdrawal thresholds of diabetic rats both in the Randall–Selitto and Dynamic plantar tests. Moreover, the falling latencies of animals did not change in the Rota-rod assessments. These results suggest that vortioxetine administration significantly improved diabetes-induced hyperalgesia and allodynia responses in the rats without affecting their motor coordination. The vortioxetine (5 mg/kg)-induced antihyperalgesic and antiallodynic effects were reversed by AMPT, yohimbine, ICI 118,551, sulpiride and atropine pre-treatments, suggesting the involvement of the catecholaminergic system, α2- and β2-adrenoceptors, D2/3 dopaminergic receptors and cholinergic muscarinic receptors in the exhibited pharmacological activity, respectively. Moreover, the data from the immunohistochemical studies indicated that the inhibition of c-Fos overexpression in dorsal horn neurons also mediates the beneficial effect of this drug. Vortioxetine induced no difference in plasma glucose levels in diabetic rats. If clinical studies confirm these findings, the concomitant beneficial effect of vortioxetine on mood disorders and its neutral activity profile on glycemic control may make it an alternative drug for the treatment of neuropathic pain.

- MakaleYalnız künye2018Scopus SJRQ2The interactions of cephalosporins on polyol pathway enzymes from sheep kidney
Şengül, Bülent; Beydemir, Şükrü
Context: Cephalosporins are derived from the fungus Acremonium. Due to their strong bactericidal ability, these drugs have to a wide usage in medicine. Objective: An investigation of the effects on sheep renal aldose reductase (AR) and sorbitol dehydrogenase (SDH) of cefoperazone, cefazolin, cefuroxime, ceftazidime and ceftriaxone as cephalosporin drugs was carried out in the present study. Methods: AR and SDH were purified from sheep kidney by ion exchange, gel filtration and affinity methods with approximately 219- and 484-fold, respectively. Some kinetic properties of the enzymes were determined such as optimal pH, optimal ionic strength, optimal temperature, stable pH, Km and Vmax. IC50 values of the drugs were found for each enzyme. Results: While the AR was inhibited by all drugs, SDH enzyme was inhibited by only CXM (IC50 8.10 mM). Interestingly, CZO activated SDH enzyme. This result was evaluated as important for the flow of the polyol reactions. Ki values and inhibition types were determined for AR. However, these values could not have determined for SDH, due to insufficient inhibition. Conclusions: From these results, it was concluded that cephalosporins may have an important effect on flow of the polyol metabolism.
