KAHRIMAN, Ilknur; AKSOY, Bahar; KANDAZ, Ufuk; BORAN, Serpil; GÖGER, Berrin; KIRBAŞ, Zila Özlem

Objectives This study investigated the effect of the puppet-assisted House-Tree-Person Test (HTP-T) on fear and anxiety levels in children undergoing chemotherapy treatment and the factors influencing these outcomes. Methods The study employed a one-group pretest–posttest quasi-experimental design combined with HTP-T analysis. The study was conducted with 29 children aged 5–12 years receiving chemotherapy at the pediatric hematology-oncology outpatient clinics of a university hospital in the Eastern Black Sea Region of Türkiye during the 2022–2024 years. Data were collected using “the Demographic Information Form for Children and Parents, the Children's State Anxiety Scale (CSA), the Children's Fear Scale (CFS), and HTP-T". Data analysis was performed using descriptive statistics, repeated measures ANOVA, and HTP-T analysis. Results Statistically significant differences were observed in children's mean CSA (F = 3.898, p = 0.026, η2 = 0.122) and CFS (F = 5.313, p = 0.008, η2 = 0.159) scores across the pre-treatment, during-treatment, and post-treatment measurements. The HTP-T analysis revealed that children had difficulty regulating anxiety and fear, expressed concerns about family and environmental events, reported feelings of loneliness, and exhibited strong emotional attachment to their families. Conclusion The study demonstrated that incorporating puppet-assisted HTP-T during chemotherapy contributed to reducing children's fear and anxiety and offered a useful approach for identifying influencing factors. Implications for nursing practice HTP-T, a therapeutic play technique incorporating puppetry and projective methods, may be effective in reducing fear and anxiety among children undergoing chemotherapy.

Kırbaş, Zila Özlem; Bayraktar, Bülent; Aktaş, Elif Odabaşı

This study aimed to determine salivary cardiac troponin I and cortisol hormone levels in primary school students to evaluate the cardiac response depending on age and gender variables. The sample of the study consisted of a total of 180 children, including boys and girls aged 8, 9, and 10 years old. The ELISA technique determined cTnI and cortisol hormone levels in saliva samples taken from the children included in the study. There was a significant difference between the salivary cTnI levels and the age levels of the children in the groups (p<0.001). A significant difference was found between the mean level of salivary cortisol hormone and the gender and age variables of the child (p<0.001). As a result of the study, a positive, weak, and statistically significant correlation was found between salivary cTnI and cortisol hormone levels (p<0.05). In the model created for the dependent variable of saliva cTnI, it was found that 7.5% of the variance was explained by the independent variables (R2 adjusted=.075), and the age variable was a statistically significant positive predictor (p<0.001). In the model created for the dependent variable of cortisol hormone, it was found that 48.4% of the variance was explained by the independent variables (R2 adjusted=.484), and age and gender were positive statistically significant predictors (p<0.001). Within the scope of the data obtained in our current study, it was determined that cortisol hormone levels were also high in children with high salivary cardiac troponin I levels. It is thought that examining and monitoring cardiac troponin I and cortisol hormone levels may be useful parameters in the nursing care and practice of children with heart disease risk and disease.

Mediating Role of Emotional Intelligence in the Relationship between Hospital Perception and Fear of Medical Procedures in Children

    Kırbaş, Zila Özlem

Aim: This study investigated the relationship between children's hospital perceptions, their fear of medical procedures, and the role of emotional intelligence as a potential mediator. Material and Method: The population of the research consists of 4th grade students (10-year-old students) studying in primary schools in the 2023-2024 academic year. The study included 343 students and employed a relational-cross-sectional-descriptive design. Data were collected using the Personal Information Form, Medical Procedure Fear Scale, Hospital Perception Scale for Healthy Children, and Ten-Year-Old Emotional Intelligence Scale. Data were analyzed with IBM SPSS V23 and IBM AMOS V24. Compliance with normal distribution was examined with the assumption of skewness, kurtosis, and multiple normality. Path analysis was used to test the mediator model, and Maximum Likelihood (ML) was used as the calculation method. Analysis results were presented as frequency (percentage) for categorical variables, mean±standard deviation, and median (minimum–maximum) for quantitative variables. The significance level was taken as p<0.05. Results: The study found that children's hospital perceptions predicted their fear of medical procedures. Specifically, there was a statistically significant positive relationship between hospital perception and fear of medical procedures (β=0.565; p<0.05). According to the mediated structural model analysis results, hospital perception had a statistically significant negative effect on the mediator variable emotional intelligence (β=-0.327; p<0.05). By including the mediator variable emotional intelligence in the model, the path coefficient between hospital perception and fear of medical procedures was statistically significant (β=0.554; p<0.05). Conclusion: In the study, it was found that emotional intelligence, the mediator variable, played a role in the relationship between hospital perception and fear of medical procedures.

Kahriman, Ilknur; Meral, Buket; Colak, Bahar; Bektas, Meral; Kirbas, Zila Özlem; Arici, Yeliz Kasko

Background Needle-related procedures are commonly performed in childhood and lead to considerable pain. Purpose This randomized controlled study was conducted to compare the effects on venipuncture pain of the Buzzy, an informational animated video, multiple interventions, and standard care in children aged 6-12 years. Methods One hundred eighty children were assigned to the groups using a computerized randomization program. Pain level was evaluated by the children, parents, and their nurses using the Wong-Baker Faces Pain Scale. The level of agreement among the nurse, parents, and children's pain reports was evaluated. Multivariable regression analysis was performed to identify the factors significantly associated with pain score. Results The participants in the Buzzy, video watching, and multiple intervention groups reported lower mean pain scores (child: KW = 28.24, p <.001; parent: KW = 18.51, p <.001; nurse: KW = 44.4, p <.001) than their peers in the control group. Moreover, age was identified as a risk factor affecting pain level (OR = 1.375, 95% CI [1.086, 1.740]; p =.008). Conclusions Buzzy and informational videos are potentially effective methods to facilitate venipuncture-related pain management in children. The use of pain management methods during venipuncture is recommended, especially in children younger than 10 years old.
