Background: Myelodysplastic syndromes (MDSs) and acute myeloid leukemia (AML) are clonal hematological disorders that share molecular origins but present with distinct clinical features. MicroRNAs (miRNAs) are key post-transcriptional regulators, and their altered expression may reflect biological shifts contributing to disease progression. Methods: Expression levels of miR-155, miR-181, miR-221, miR-222, and miR-223 were analyzed by RT-qPCR in bone marrow samples from 37 MDS patients, 20 AML patients, and 7 controls. Group comparisons were performed using ANOVA (with Benjamini–Hochberg correction) and Tukey post hoc testing. Diagnostic performance and network behavior were evaluated using ROC analysis, Pearson correlation matrices, and principal component analysis (PCA). Results: miR-155, miR-181, and miR-223 were upregulated in AML, whereas miR-221 and miR-222 were downregulated. miR-222 showed the highest diagnostic accuracy (AUC ~0.87 for both AML vs. control and MDS vs. control). Its expression was significantly higher in high IPSS-R MDS cases (p = 0.046), with a similar upward tendency for miR-221 (p = 0.054). Progressive loss of coordinated miRNA expression was observed from controls to MDS and AML. PCA supported these findings by showing separation mainly driven by miR-222 and miR-155. Conclusions: Combined miRNA profiling highlights miR-222 and, to a lesser extent miR-155, as consistent indicators of myeloid disease transformation. While further validation in larger and genetically stratified cohorts is warranted, these findings support the potential contribution of miRNA signatures to diagnostic evaluation and risk stratification in MDS and AML, in line with precision hematology approaches.

The Relationship Between Metacognitive Beliefs and Salivary Cortisol, BDNF, and NDNF Levels: A Cross-Sectional Study

Okur, Süheyb; Bayraktar, Bülent; Tosun Köse, Fatma

Purpose: The main objective of this study is to non-invasively investigate the relationship between metacognitive beliefs and cortisol, the primary stress output of the hypothalamic-pituitary-adrenal (HPA) axis, as well as neurotrophic factors associated with neuroplasticity brain-derived neurotrophic factor and neuron-derived neurotrophic factor (BDNF and NDNF). Within this framework, the hypotheses that negative metacognitions would be associated with increased cortisol and decreased BDNF levels, and that cortisol might play a mediating role in this relationship, were tested. Method: The study was designed in a cross-sectional model with 60 university students. Participants' metacognitive beliefs were measured using the Metacognitions Questionnaire-30 (MCQ-30). Salivary cortisol, BDNF, and NDNF levels were analyzed using the ELISA method. Pearson correlation and hierarchical multiple regression analyses were used for data analysis. Finding: The results showed a significant positive relationship between the total metacognition score and cortisol (r = 0.589, p < 0.01) and a strong negative relationship between cortisol and BDNF (r = −0.662, p < 0.01). Hierarchical regression analysis supported a partial mediation model, indicating that dysfunctional metacognitive beliefs have both a significant direct negative association with BDNF and an indirect association mediated by cortisol. In the final model, both metacognition (β = –0.298, p < 0.05) and cortisol (β = –0.281, p < 0.05) were significant factors associated with lower BDNF levels. NDNF showed a positive relationship with BDNF (r = 0.571) but not with other variables. Conclusion: These findings point to a psychobiological model where dysfunctional metacognitive beliefs are linked to suppressed neuroprotective mechanisms like BDNF, both directly and indirectly through HPA axis activation. The results shed light on the potential neurobiological mechanisms underlying the effectiveness of metacognitive therapies.

Breast cancer (BCa) is one of the most prevalent cancer amongst women and PIK3CA mutations have a crucial role in carcinogenesis. On the other hand, Octamer-Binding Transcription Factor 4 (OCT-4) is common biomarkers of cancer stem cells, which provide to the pathological processes of numerous carcinomas. In this study, we evaluated the association of OCT-4 gene expression levels with the PIK3CA mutation in BCa patients. The mRNA expression level of the OCT-4 gene was detected by qPCR in 44 BCa patients and 30 BCa patients control have positive PIK3CA mutations. Exon 9 and exon 20 of PIK3CA mutations were identified by sequencing. OCT-4 gene expression and PIK3CA mutations were also clinically evaluated in BCa patients. The frequency of PIK3CA mutation in 44 new cases were 32%. PIK3CA mutations were significantly associated with HER2-positive (p = 0.0271) status, Ki-67 status (p = 0.0434) and patient age (p = 0.0149). OCT-4 mRNA expression was significantly higher in the BCa tissue in comparison to normal breast tissue (p = 0.0305). A significant correlation was found between expression of OCT-4 and node status (p = 0.0190). OCT-4 mRNA expression was higher in the PIK3CA mutation tissue than the wild type tissue. However, no association was found between the expression of OCT-4 and the PIK3CA mutation or wild type samples (p = 0.7908). Our results provided that OCT-4 gene and PIK3CA mutations may have a role in development of BCa. OCT-4 mRNA expression level and PIK3CA mutations have a potential important for BCa diagnosis or treatment.
